Through one of life’s most difficult times, our care team works together to ensure that the physical, emotional and spiritual needs of the patient are met.
Oakland Hospice specializes in end of life care. By managing symptoms and controlling pain, we give our patients the opportunity to enjoy the highest quality of life for as long as possible. Our specialized care offers patients the chance to share more special moments with their family and express all the things they need to say.
Hospice services are delivered wherever the patient calls home. If you wish to remain in your family home, we’ll come there. If you’re in an assisted living community or nursing home, we can partner with the facility to provide extra, specialized end of life care.
While receiving hospice care, the patient will receive visits from their personal team of healthcare professionals including nurses, aides, social workers, chaplains and volunteers. Each individual has been trained to meet the patient’s physical, mental, emotional, spiritual and personal care needs. The patient’s hospice team will help with bathing, administering medication, wound care, companionship, and more.
In addition, all medication, medical equipment and medical supplies related to the patient’s terminal illness will be provided at no cost to them or their family. Contact us now for additional hospice information for patients.
